Vaccination Status
With a large portion of our student body now eligible to be vaccinated, we are excited to expand our list of vaccinated students. Please complete this updated Google Form or email admin@urbanprairie.org with your student’s vaccination dates!
2021-22 Student Health Information
All students must be up-to-date with their physical examination, immunization requirements, and other completed forms. These forms must be turned into the office by October 4, 2021. Forms are available from your child’s doctor/dentist/optometrist or from Administration. Your doctor should have these forms on file, and you can find them here:
Allergy/Asthma Action Plans
If your child has an Allergy / Asthma Action Plan please check with admin@urbanprairie.org to ensure that we have the most up-to-date information and medication. Medication should be sent to school in a zip lock bag labeled with your child’s name. This will stay with the class in their park bag as to be in reach at all times.

Please photograph and upload your health forms in Operoo, or drop them with Administration. Requirements by age/grade:

Immunization: Early Childhood (submit annually), new/transfer students, students entering Kindergarten, 1st, and 6th grades.

Medical Examination: Early Childhood (submit annually), new/transfer students, students entering Kindergarten, 1st, and 6th grades. Physical must have been completed within one year prior to October 1, 2021.

Dental Examination: Students entering Kindergarten, 2nd, and 6th grades, and all new/transfer students. Due April 1, but encouraged sooner. Examinations must have been completed within 18 months prior to May 15, 2022.

Eye Examination: Students entering Kindergarten, new/transfer students. Examinations must have been completed within one year prior to October 15, 2021.
